Project Summary
East London Foundation Trust (ELFT) provides a wide range of community and inpatient services to children, young people, adults of working age, older adults and forensic services to the City of London, Hackney, Newham, Tower Hamlets, Bedfordshire and Luton. They also provide psychological therapy services to the London Borough of Richmond, Children and Young People's Speech and Language Therapy in Barnet and specialist addiction services in Redbridge.
The SMI query reconciles ELFT secondary care mental health data with primary care SMI datasets to make sure that:
- Primary care data systems are updated with diagnostic data from ELFT’s secondary care dataset regarding SMI diagnosis.
- Care for patients with an SMI and discharged from ELFT’s secondary care services is transferred to primary care.
- Improved monitoring of the physical health of SMI patients between primary care and secondary care.
- GP practices are assisted in delivering the NHS England policy Improving physical healthcare for people living with severe mental illness (SMI) in primary care:

Patients registered with City & Hackney, Newham and Tower Hamlets localities within North East London ICB.
Project cohort
All patients with an ICD-10 diagnosis of: F10.5, F11.5, F12.5, F13.5, F14.5, F15.5, F16.5, F19.5, F20-29, F30.2, F31.2, F31.5, F32.3 and F33.3.
Project criteria
Patients diagnosed with a Serious Mental Illness (SMI) who are on each GP practice’s primary care QoF register.
